| Established in 1965 by Caddo Parish, North Caddo Medical Center offers a rare combination of advanced healthcare capabilities and provides exceptional care to all the families it serves. North Caddo Medical Center is proud to be a community owned facility. Operating as a critical access hospital with 15 acute care beds and 10 additional long term care beds the hospital serves the villages and towns in their district: Vivian, Oil City, Mooringsport, Belcher, Dixie, Gilliam, Hosston, Ida, Mira, and Rodessa. Surgery, cardiac care, diagnostic radiology, labor and delivery, pediatrics and emergency medicine are among the many services that are offered at North Caddo Medical Center. Two fully- equipped ambulances that provide 24- hour emergency transport are also available to the members of the hospital service district. North Caddo Medical Center health system also includes the North Caddo Medical & Surgical Clinic, the Plain Dealing Medical Clinic, the North Caddo HealthPlex, and the North Caddo Fitness Center. The staff includes five full-time physicians, two nurse practitioners, and a physician's assistant. About Dr. HaynesJohn H. Haynes, Jr., M.D, has served as Chief of Staff for North Caddo Medical Center and as a Board Certified Family Practitioner at The Medical & Surgical Clinic, in Vivian, LA., since his arrival in 1966. Dr. Haynes received his Bachelor of Science Degree from Centenary College of Shreveport and his Medical Doctorate from the University of Texas Medical School in Galveston, Texas. He completed his internship and residency training at John Peter Smith Hospital. Dr. Haynes participated in neurology and hematology research at the University of Texas Medical Branch and cardiology research at John Peter Smith Hospital, in Fort Worth, Texas. Dr. Haynes has served on the Caddo Parish School Board, and was instrumental in establishing the National Teachers Exam minimum score as standard for teaching in the parish and the state, and for providing teacher incentives in increased pay for educators to travel into rural areas to teach. In 1993 Dr. Haynes, in conjunction with Willis Knighton Health Systems and Louisiana State University School of Medicine, founded and directed a Rural Family Practice Fellowship Program involving post graduate training in rural areas. He also serves as the Director/Coordinator of the Family Practice Residency Rural Tract for Louisiana State University Health Science Center Shreveport. In November of 1993, Dr. Haynes was selected from a field of three hundred rural physicians to be the first "Country Doctor of the Year." This award was created to recognize the outstanding contributions to healthcare made by America's rural physicians. Dr. Haynes was honored by being named Louisiana Family Practitioner of the Year in 1998, and 2004. He received Louisiana's Rural Practitioner of the Year award in 1998 and again in 2009. About Dr. TaylorStephen G. Taylor, M.D., began serving rural Northwest Louisiana in 1986 as North Caddo Medical Center's Assistant Chief of Staff, and as a Family Practitioner for The Medical & Surgical Clinic. Dr. Taylor is Board Certified in Emergency Medicine, and Family Practice, along with a Certificate of Additional Qualification in Geriatrics. His extensive education credits include: a Bachelor of Arts degree in mathematics and physics from Vanderbilt University, a Master of Science degree from the University of Florida, and a Medical Doctorate and internship with the Medical College of Georgia. He completed his residency training at John Peter Smith Hospital in Fort Worth, Texas. Dr. Taylor's vast medical experience in rural family practice allows him to serve this area in many capacities. He currently serves as Director of Emergency Services for NCMC, as Chairman of the North Caddo Health Care & Education Alliance, Co-Chairman for the Medical Home Committee of Louisiana Healthcare Quality Forum, Executive Committee for Future of Family Medicine Task Force, and as Associate Director of the Rural Tract Family Practice Residency Program, for Louisiana State University Health Sciences Center. Dr. Taylor was elected as Mayor of the Town of Vivian, LA, in 2003, and remains in office today. His local and state wide involvement contributes to increasing awareness for the needs of the families in our rural communities. He is the Medical Director of North Caddo Parish Fire Districts 1, 7, & 8, a member of the Board of Directors for Coordinating and Development Corporation, and sits on the Legislative I-49 Task Force. Dr. Taylor has also been published as the co-author of, Laparoscopic Cholecystectomy in a Rural Family Practice: The Vivian, LA, Experience, The Journal of Family Practice, 2004. Dr. Taylor has been an honored recipient of the Preceptor of the Year-Family Practice Residency award, Outstanding Man of the Year by the Vivian Chamber of Commerce, Ministerial Alliance- Community Service Award, Country Doctor of the Year finalist- 2008, as well as being named The Louisiana Rural Practitioner of the Year, 2008. |

About Dr. LococoVincent B. Lococo, M.D. joined North Caddo Medical Center and The Medical & Surgical Clinic in 2000, as a key team member of the NCMC team of Family Practitioners. Dr. Lococo began his medical career after serving three years on active duty with the United States Army from 1983- 1986. He earned his Bachelor of Science Degree at the Louisiana State University of Shreveport, in 1989, and continued serving his country in the U.S. Army National Guard, while acquiring his Medical Doctorate at Louisiana Statue University School of Medicine in Shreveport in 1993. Dr. Lococo concluded his formal education with the completion of his Family Practice residency program at Louisiana State University Medical Center in 1996, and retired from the U.S. National Guard in 2005. His current certifications include: American Board of Family Practice recertified, Advanced Cardiac Life Support, Advanced Life Support in Obstetrics, and Advanced Trauma Life Support. Dr. Lococo is a member of American Academy for Family Practitioners, the Louisiana Medical Society, and the Shreveport Medical Society. Today, Dr. Lococo serves patients throughout a large part of Ark-La-Tex. He practices Family Medicine full-time at NCMC and The Medical & Surgical Clinic in Vivian, LA., as well as serving as the Medical Director for Bradley Medical Clinic in Bradley, Arkansas, the Whispering Pines Nursing Home, Physician's Behavioral Hospital of Shreveport, Red River Behavioral Hospital, Bossier City, LA, Life Path Hospice, Pilgrim Manor Nursing Home, as well as Vivian HealthCare Nursing Home in Vivian, Louisiana. |

About Dr. ChanceJody D. Chance, M.D., has been an important part of North Caddo Medical Center (NCMC) and The Medical & Surgical Clinic since 2006. He began pursuit of his medical career at Louisiana State University, where he received the Award for Excellence in Undergraduate Research and graduated Cum Laude with a Bachelor Degree of Science in Biology. Dr. Chance continued on with Louisiana State University Health Sciences Center in Shreveport (LSUHSCS), where he earned his Medical Doctorate. After completing one year of his residency in rural family medicine at LSUHSCS, he joined the NCMC team, and completed the remainder of his residency program in 2008, as Chief Resident. Dr. Chance is Board Certified in Family Medicine, he is an instructor for Advanced Cardiac Life Support and Pediatric Life Support, and has been certified by The American College of Surgeons in Advanced Trauma Life Support. Dr. Chance is proud to have served his country in the National Guard for eight years, and received an army accommodation medal for his "devotion to duty and outstanding performance," along with an honorable discharge. |

About Dr. McCommonNicole A. McCommon, M.D., came to North Caddo Medical Center and The Medical & Surgical Clinic in Vivian, Louisiana, in 2005, with the Louisiana State University Health Science Center -Shreveport, Rural Tract Program. She is no stranger to the medical needs and issues of the citizens of rural communities, having seen them first hand in her hometown of Elmer, LA. Dr. McCommon began her medical education at Louisiana State University in Alexandria, LA., where she acquired her Associates in Science degree. She was a member of Gamma Beta Phi, received the TH Harris Scholarship, and received recognition of her academic achievements in 1994, by winning the Academic Excellence Award for Chemistry and the Academic Excellence Award for Biology as well. Dr. McCommon then attended Louisiana State University in Shreveport, LA, and graduated Cum Laude with a Bachelor Degree in Biochemistry, with a minor in Chemistry in 1998. She earned her Medical Doctorate from the Louisiana State University Health Sciences Center (LSUHSC) in Shreveport, in 2005, and received the esteemed Family Medicine of the Year Award and the Leonard-Tow Humanism in Medicine Award. Dr. McCommon completed her first year of Residency Training at Louisiana State University Health Science Center in Shreveport. Dr. McCommon finished her last two years of Residency Training in the Rural Tract Residency Program at NCMC. In 2007 she was named Chief Resident for the program. Besides volunteering for medical causes such as Breast Cancer Awareness, Parkinson's disease, and Multiple Sclerosis, she has also been active in multiple youth benefit organizations since 1993. Dr. McCommon is a Clinical Assistant Professor of Family Medicine, Gratis, in the Department of Family Medicine and Comprehensive Care at LSUHSC. We are privileged to have Dr. Nicole A. About Sean Landreneau
Sean Landreneau, RFNP, has been a large part of North Caddo Medical Center and The Medical & Surgical Clinic since 2004. Ms. Landreneau began serving rural Louisiana as a Registered Nurse after earning her Bachelors of Science Degree from Southeastern Louisiana University in 1985. After eleven years as a Registered Nurse in surgery and emergency rooms throughout the area, Ms. Landreneau utilized her invaluable clinical experiences to earn a Master's of Science Degree from Northwestern Louisiana University in 1996. She is certified by the American Nurses Credentialing Center (ANCC) as a Family Nurse Practitioner and has completed courses in Advanced Cardiac Life Support, Pediatric Advanced Life Support, and Basic Life Support. Ms. Landreneau has a strong dedication to helping those in need, not only in her medical practice, but in her community outreach programs as well. For over twenty years, she has coordinated and directed student ministry programs, including international, national, state, and local mission events to serve the underprivileged. For 24 years, Ms. Landreneau has provided excellent, patient-centered, and compassionate care, with an extensive focus on disease prevention, health education and counseling for people of all ages and cultures. |

About John Messier
John Messier, PA-C, has been a Primary Care Provider at NCMC since 2000. He began his medical journey as a respiratory therapist 18 years ago. He has earned two Bachelor's Degrees from Louisiana State University Health Science Center (LSUHSC), in Cardiopulmonary Science and Physician Assistant Studies. His Master's of Physician Assistant Studies degree is from UNMC with a specialization in Family Medicine. Awards and honors John received in school include; The Louisiana Society for Respiratory Care Award of Achievement, Dean's Award Nominee Department of Cardiopulmonary Science, Ronald B. George Award for Clinical Excellence, and the Joseph K. Valgora Award for Compassionate Caregiver. He has served as a Captain in the Louisiana Army National Guard for 14 years. John's dedication to his country and his patients has been recognized by the following awards and honors: Combat Medical Badge-Operation Enduring Freedom V, Meritorious Service Medal, Army Commendation Medal-3rd, Afghanistan Campaign Medal, Humanitarian Service Medal, Louisiana War Cross, and National Defense Service Medal with Bronze Star, Global War on Terrorism Expeditionary Medal, Task Force Katrina Service Medal, and the Louisiana Emergency Service Ribbon. Today, John provides primary care to patients of all ages in NCMC's Medical & Surgical Clinic. About Kathleen Richardson
Kathleen Richardson, RFNP, has practiced in collaboration with North Caddo Medical Center's (NCMC) John H. Haynes, Jr., M.D., since 1997, coordinating the care of patients in the rural Ark-La-Tex by encompassing all aspects of routine care. Ms. Richardson joined North Caddo Medical Center's Medical Clinic- Plain Dealing, in January, 2009, bringing with her 23 years of experience as a Registered Nurse, and 7 years as a Family Nurse Practitioner for the surrounding rural communities. She began her medical career after graduating in 1979 with her Bachelors of Science in Nursing, from Northwestern State University, Shreveport, Louisiana, and then earning her Masters of Science in Nursing in 1995, also from Northwestern State University, Shreveport. Practicing as a Family Nurse Practitioner in rural health clinics for 7 years, she is certified by American Nurses Credentialing Center (ANCC) and has had experience with all age groups and practices different specialties, including, family medicine, pediatrics, geriatrics, urology, and gynecology. Ms. Richardson has contributed to her passion for promoting preventative health care by coordinating and implementing the Kid Med program that provides preventative health care services to Medicaid patients from 0-21 years of age. Ms. Richardson can be found in her free time giving sports physicals to rural schools, holding Teddy Bear Clinics for pre-school age children of the area, and donating her time and medical talents to health fairs in the Ark-La-Tex to raise awareness for the benefits of health screenings, nutrition, and exercise for preventative care. |
